Job Description

I need a sql database which could be updated by entering data into fields and could be searchable by a small number of inputs. The database would need to contain the following data:
Business Name
Contact Name
Address
Phone number
Fee description (three categories)
\"About Us\" description

This database would reside \"behind the scenes\" of a website I\'m developing. I want members to be able to add data through a form which would ask for the above data. I want the consumer to be able to enter zipcode or state and have a list returned reflecting the businesses within said requirement. This is for a firearms website operating in the US. A good example of the end goal can be found at the following link under the \"Search our FFL holder Network\" heading:
