IPhone and android multiple choice question app

I would like someone to develop a multiple choice question application for iphone and android (must be honeycomb compatible).

There will be 550 - 600 five stem multiple choice questions. These will all be true or false. Every question will have a little explanation (usually within 150 words and may or may not have a graph, diagram, table...etc etc). So, an example of a single multiple choice question would be as follows.

Drug X causes the following

A - side effect 1
B - side effect 2
C - side effect 3
D - side effect 4
E - side effect 5

Explanation (up to 150 words)

All questions will be submitted in this format as a word document. Each question will be uniquely numbered by us.

When the user answers the question they get the explanation as well as being told what they got right and wrong and their score (both for the question and cumulative for this session).

I would like the developer to design the app from the ground up (we have no design work) and I would like to have a method of maintaining the question database independent of the developer. Ideally I would like a web based service so I can access it anywhere. I want the ability to edit and alter the number the question pool over time to ensure accuracy.

The questions will focus on four categories and I would the user to be able to do questions within any combination of these topics (e.g topic A exclusively, topic B and C... etc etc). I would also like people do random questions and to be able to do customisable questions. The customisable questions will involve them setting a number of questions and the app taking randomly from the pool of questions from the chosen topics. Finally, I would like the questions cut up into 5 equal size exams so the user can do proper exam practice. This way people can do the 5 exams and cover all of the available content. If the question bank increases (see below for submit a question) I would like the ability to increase the number of exams and to be able to alter what questions are in each exam.

Additional features the app must have include the ability to report a problem with any question and submitting a question. When reporting the problem this will send an email saying there has been a problem with the question number and a little bit about what the problem was. If people hit submit a question then it will show them the appriopriate email address send them and the types of questions I am after. I would like to able to edit this screen as I will be requiring different questions submissions at different times. These submissions must be fully inclusive and be able to add them to all parts of the app.

The app should have an about us section to explain the story of the app (it is a little unique) and give the necessary author, editor, lead credits.

In our timeframe the entire department is working on questions and we will have a completed question pool by the middle to end of october. So really we need to get moving with the software development.

In all quotes I would like total cost (iphone + android + maintenance), any ongoing costs and the level of support included in this quote. I will also require a time frame for the following factors

1 - time to develop the software
2 - time to add the questions

I must be able to thoroughly test the software before it is generally released so you must have some form of beta'ing software for both iphone and android.
