HTML form to populate PDF then email and store.

I am looking for someone to run me up an automated HTML form to populate a PDF and then send the PDF to an email address.

I will provide you with a PDF with form fields and also a list of these fields. The PDF will be in the format of a quote or contract.

am using PHP 5.1 / mySQL

An HTML/php form on submit will,

1. Calculate the cost. (cost sheet provided) 
2. Populate the fields in the PDF.
3.Store the fields in a MySQL table.
4.Send the PDF and a customisable email

The costs should be easily editable for futureproofing.


so again, to summarise,
a user visits a website, enters for example, 4 computers, 1 server and 1 printer as a numeric value, and on submit, a thank you - you will receive your quote to your email address within x minutes.

The php file will calculate a monthly cost (or minimum cost of lower) and poplulate the pdf fields. This will then be emailed to me and the customer.
