Flash uploader required

I need an uploader written in Flash, it needs to work in a specific way:

Make initial request with the current session cookie and the size of the file being uploaded.
Upload raw binary file using PUT with to a URL returned from the above request.
Call a completion URL with some additional fields (email address(es), message), these fields come from a form so can be accessed via JavaScript.

I don't know much about Flash so if any of that is not possible, perhaps you can suggest an alternative.

It must have a specific look and feel or have JS callbacks to update the UI via jQuery.
