Design and Build of Web Form

Hi,
I require a form to built for my website so that potential customers can send me their quotation requests.
The form needs to be designed to look professional and programmed to work consistently and on most platforms.
I have designed a working prototype in asp to show the functionality and my design ideas.
The prototype form is missing important features like validation, save function, postcode lookup and a couple others.
