Database / Spreadsheet / Mobile Script

I'm running a project for a website and there are about 8 forms in PHP that email the data to my email. I need to have these data also stored into a database and excel spreadsheet.

Database needs to have the option to add/edit/remove/view and print into A4 paper options. The mini cPanel will need to have a logo that I will provide.

Excel spreadsheets will also have the data on it in a Google Docs form like. So we can work on statistics later on.

I'm also looking for a full code that redirects to my mobile version and from the mobile version to the full site.

The website is in Portuguese BUT you don't need to know Portuguese at all, just need to copy and paste the fields. Please provide screenshots of previous works so I can have an idea of how you work.

It is essential to keep a good communication throughout the project and to have few samples of your work on the portfolio tab on peopleperhour.
