Connecting database into website

Need to connect database to website. person will submit name email and message on the website. Upon submission Info will be stored in the database. Thats literally all.
Im short on time , this is for school simple project, and I am not into buliding those things.

Must be done in mySQL, any additional things that you could provide for the website with some simple Javascript features would be big plus.

Will sent you website files, just provide me with a rate. Shouldnt take more than an hour of work literally.
