Application to pull reviews from amazon

I would like a small web-based application, purely to pull in review data for amazon products. Ideally this would pull in the complete review information for the product (ie. all 250 reviews if the product has 250 reviews).

The data I would like it to pull in is:

1. Review rating.
2. Review title.
3. Review date.
4. Reviewer name.
5. Link to reviewer profile.
6. (not essential) number of reviews from this reviewer.
7. (not essential) location of reviewer.
8. (not essential) number of helpful votes on the review.

I would like it to work as follows:

1. There would be a page containing a text field for me to enter the Amazon product ID. A submit button would then query the Amazon database. (ideally, but not essential, there would be radio buttons for UK/US reviews)
2. A page would then display all review information in a table on the following page.
3. (not essential) it should be possible to click a button to export the info to Excel.

If it's in PHP that's great, if you think something else would be better, feel free to propose that.
