Add a Flash contact form to a web video

I am about to commission a video for the home page of my company's website. The video production company will give me the video file.

However, at the end of the video, I would like an enquiries form to appear over the background of the last frame, to allow people to enquire immediately, rather than clicking on the "Contact" page. When completed, the contact form should email me the details.

I would like to get quotes for producing the .swf and .fla files in order to allow me to integrate this with my website.
