3 web pages plus php quote form, coding only

I am redeveloping a website for a client and time is of the essence so I am interested in getting someone develop about 4 pages for me.

I need:

Dreamweaver CS4 template encompassing header, footer and editable content area.
Subsequent pages are to be saved as .php to allow for future php compatibility, although the coding will only be html and css at this stage.
Default.php
Subpage.php - almost identical to default.php, only difference is the text content.
Online quote form which includes checkboxes, text input areas and drop down lists, must submit info using php


I also need Meta Tags and specific titles to be added to the pages that you develop (I will supply all documentation).

I need the main drop down menu to be CSS only, no javascript, for SEO functionality. This is very, very important.

My client and his SEO company are going to have access to the code and cannot know that I have outsourced this as it will impact future work I get from them so I need client confidentiality and no references to your organization in the code at all.

Code needs to be clean, validated, semantic, fully tested on all major browsers and well marked up so that I can implement it on subsequent pages.

The site is actually fairly simple and I do not anticipate and problems for anyone who is experienced.

I can supply full documentation, visual reference files (photoshop), all required assets, as well as the existing html and css files which i have begun coding. I can also supply the url of the existing website for reference.

I am looking for someone who is very experienced in html and css as well as php (for the form). This job is URGENT so I am looking for someone who can begin immediately.

I am hoping for a turn around time of 2-3 days as I have a very tight deadline as the site will go live on 23 March. I am happy to take receipt of the template and default page in the meantime, while you continue work on the sub page and the form.

I am also hoping to develop a good relationship with someone who can help me out on other work.
