A complete Electronic Patient Record database system to manage appointments and assessments built for a clinic specializing in screening for skin cancer. The system allows nurses to post high definition images of skin lesions and add clinical notes. The system is then accessed remotely by dermatologists who can examine the images and notes and make diagnoses and recommendations. Patients receive detailed printed reports which are also stored in PDF format on the system. Reports were mostly printed in situ but could also be automatically faxed to GPs if required. The database was initially built using FileMaker Pro then after approx one year of use in the busy clinic, it was ported to a web application with complete migration of all legacy data. I designed and built the FileMaker database, gave ongoing support during its use in the clinic, ran management searches and reports, assisted with data migration and advised on security and backup.

Holocaust Memorial Day Trust (www.hmd.org.uk) is a Human Rights charity. In 2009 I built a database system to allow their small office with a national remit to deal efficiently with the huge numbers of requests for campaign packs, to generate mailings and to track correspondence.

The system was built using FileMaker Pro and featured an internal messaging system, tailor-made data input and "quick search" screens, customized labels automatically selected for different sizes of packs, and different levels of access for staff members controlled by user login accounts.

I carried out the migration of over 10,000 legacy records from over 30 excel spreadsheets, assisted with the preparation of searches and reports and provided ongoing support and training for users.
